DESCRIPTION The Pharmacist II in the Nephrology Clinics is responsible for providing direct patient care to patients with nephrology-associated conditions at UC San Diego Health. The pharmacist will conduct virtual and/or in-person visits and work under a Collaborative Practice Agreement to provide comprehensive medication management services, including medication optimization, laboratory monitoring, patient education, medication access support, and chronic disease management. The pharmacist will collaborate with providers, pharmacy staff, and interdisciplinary teams to improve clinical outcomes, address healthcare gaps, and ensure safe and effective use of medications across the continuum of care. The pharmacist in this position may also serve as a preceptor to pharmacy students and residents as assigned and complete other activities at the discretion of managers and supervisors. MINIMUM QUALIFICATIONS Doctorate of Pharmacy Degree; or equivalent clinical experience. Registered Pharmacist issued by the state of California (RPH). Incumbents can meet the minimum experience qualifications for this position in 1 of 3 ways: 1) possesses a PGY 2 or 2) possesses a PGY 1 plus 1 year of pharmacist experience, or 3) possesses 4 years of pharmacist experience. PREFERRED QUALIFICATIONS Pharmacy residency training or equivalent clinical experience. Board certification in relevant clinical specialty. Direct experience treating patients with nephrology-associated disorders. SPECIAL CONDITIONS Must be able to work various hours and locations based on business needs. Pay Transparency Act Annual Full Pay Range: $200,010 - $238,972 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $95.79 - $114.45 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
