We are seeking two *Housing Stabilizers (Case Managers) *to provide intensive support services to participants in our Scattered Site Permanent Supportive Housing (PSH) Program and Time-Limited Subsidy Program.
Position Overview The Housing Stabilizer plays a critical role in supporting individuals and families as they transition from homelessness to stable housing. This position provides intensive case management, housing stabilization services, crisis intervention, advocacy, and resource coordination for participants facing challenges related to mental health, substance use, medical conditions, income instability, and other barriers to self-sufficiency. The ideal candidate is compassionate, resilient, solutions-focused, and skilled at building trusting relationships with vulnerable populations while maintaining professional boundaries and strong documentation practices. Team Structure * Supportive housing community with approximately 80 units * Collaborative team of 4 Case Managers * Reports directly to the Housing Stabilization Manager * Caseload of approximately 20-25 individuals and families Key Responsibilities * Provide intensive case management and housing stabilization services to individuals and families. * Conduct client assessments, identify service needs, and develop individualized service plans and goals. * Maintain timely and accurate case notes, client records, and database documentation. * Coordinate services and connect participants to housing resources, healthcare, mental health treatment, substance use treatment, public benefits, employment services, and other community resources. * Provide crisis intervention, de-escalation, safety planning, and ongoing support during challenging situations. * Advocate on behalf of clients with landlords, service providers, and community partners. * Support participants in developing independent living skills, budgeting, problem-solving, and self-sufficiency. * Promote community integration and long-term housing retention. * Participate in case conferences, team meetings, trainings, and professional development opportunities. * Assist with resident engagement activities, community-building events, and other housing program initiatives. Required Qualifications * Minimum of 1 year of case management, social services, behavioral health, housing services, or related experience. * Experience working with individuals experiencing homelessness or housing instability. * Strong organizational skills and ability to manage multiple priorities in a fast-paced environment. * Experience maintaining case notes, client documentation, and data entry systems. * Ability to communicate effectively with clients, landlords, community partners, and multidisciplinary teams. * Proficiency with Microsoft Office Suite and electronic documentation systems. Preferred Qualifications * Experience supporting individuals impacted by: * Substance use disorders * Severe mental illness * Trauma * Chronic medical conditions * Knowledge of Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care principles. * Experience conducting risk assessments, crisis intervention, and conflict resolution.
