The Practice Care Coordinator role assists in coordinating the organization's clinical care management and Primary Care activities, including care transitions activities to help improve the care outcomes for eligible at-risk patients. Responsible for supporting primary and specialty care objectives in outreach to patients, coordination of care with managers, and proactively creating relationships with physicians/advanced practice practitioners and patients to ensure optimal health and wellness for the patient population.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education:. Graduate of an accredited Medical Assistant program OR completion of EMT-Basic, EMT-Advanced or Paramedic training required.
  2. License/Certifications: New Hampshire Residents must have National Registry AEMT or Paramedic License and/or NH AMET or Paramedic License. Maine Residents must have National Registry AEMT or Paramedic License. Certified Medical Assistants must have certification and a New Hampshire Tech License. Current BLS certification required or must obtain within 30 days of start date.
  3. Experience: One year of experience in a medical facility as a Medical Assistant and/or documented evidence of an externship completed in a medical facility required.
