As a Case Manager, Clinical Services, you will coordinate care and services for individuals coping with mental health challenges by providing education, coaching, advocacy, and resource coordination. You will help clients navigate healthcare systems, strengthen coping skills, and access community supports while promoting the important connection between physical and mental wellness. This position offers the opportunity to work in a variety of settings including the clinic, community, client homes, and telehealth environments, making each day both meaningful and dynamic.

Key Responsibilities:
* Coordinate behavioral health and primary care services to support clients in achieving improved overall wellness and recovery goals
* Coach and educate clients on navigating healthcare systems, accessing resources, and strengthening independent living skills
* Build strong therapeutic relationships with clients and families through supportive case management and advocacy
* Utilize motivational interviewing, crisis intervention, behavioral management techniques, and active listening to support positive outcomes
* Develop, implement, evaluate, and update individualized treatment plans in collaboration with clients and their support systems
* Connect clients with community resources, support services, and natural support systems to reduce barriers to care
* Provide services in multiple settings including office, community, telehealth, and client homes based on individual needs
* Complete assessments, attend staffing meetings, and actively participate in clinical supervision and team collaboration
* Maintain timely and accurate documentation within the Electronic Medical Record system for all services provided
* Demonstrate ethical and professional practices when working with clients, families, and community partners
* Maintain productivity expectations and complete all required agency trainings and compliance activities

Education, Experience, and/or Credential Qualifications:
* Bachelor's degree in psychology, social work, counseling, or related human services field required
* Experience working with Electronic Health Record systems preferred
* Experience supporting individuals with mental health, behavioral health, or community-based services preferred
* Knowledge of case management principles, behavioral health services, crisis intervention, and recovery-oriented care preferred

Additional Qualifications:
* Must possess a valid driver's license in good standing
* Ability to travel within the community to meet with clients and community partners as needed
* Strong communication, organizational, time management, and problem-solving skills required
* Ability to work independently while collaborating effectively within a multidisciplinary team
* Comfortable providing services in office, community, home-based, and virtual environments
* Demonstrate professionalism, empathy, cultural awareness, and a commitment to person-centered care

Physical Requirements
* Frequently sit for extended periods while completing documentation and client coordination activities
* Occasionally stand, bend, kneel, squat, lift, and drive as part of daily responsibilities
* Ability to travel occasionally within the community for client visits, meetings, and care coordination
* Ability to safely perform light physical activities associated with community-based case management services

Pay Range:

$19.10--$26.80
