Position Information

This recruitment is promotional only. You must presently be a full or part-time regular or probationary County of Sonoma, Community Development Commission, Sonoma County Fairgrounds, or Sonoma County Water Agency employee in order to apply for this position.

The Department of Health Services (DHS) has requested a department promotional certification to fill a Bilingual Senior Public Health Nurse (English/Spanish) position. Only qualified permanent and/or probationary DHS employees on the eligible list may be eligible for referral to the department for the advertised position. This employment list may also be used to fill future full-time or part-time positions as they occur during the active status of the list.

Housed within the Human Services Department's Adult and Aging Division, the Multipurpose Senior Service Program (MSSP) provides social and health care management for older adults who are certifiable for placement in a nursing facility, but who wish to remain in the community. The goal of the program is to arrange for and monitor the use of community resources to prevent or delay premature institutional placement of these clients.


The MSSP Senior Public Health Nurse (Senior PHN) plays a lead role in the local administration of this state-wide care management program by assuring that each client meets the level of care certification required annually for the program, as delineated by the California Department of Aging and the Department of Health Services. The MSSP Senior PHN is responsible for conducting home visits that include comprehensive geriatric assessments (initial and annual reassessments) for all MSSP clients, addressing physical, cognitive, functional, psycho-emotional, and nutritional needs. This Senior PHN position will serve the MSSP program and will also provide support to other Adult and Aging programs. The MSSP Senior PHN works independently and in close partnership with the MSSP Social Work team. The Senior PHN triages priorities and can plan, organize, and direct the work of social workers and other personnel in home-based and office settings.

Please note: This bilingual position requires English and Spanish skills at the fluent (reading, writing, and conversational) level. Applicants who wish to be considered for a bilingual position must indicate their level of skill in the required supplemental questionnaire. An examination will be conducted prior to employment in a bilingual position to confirm the level of skill in this area. Bilingual premium pay is $1.50/hour for fluent skill sets.

About the Team

The Adult and Aging section of the Department of Health Services is a tight-knit team of dynamic and talented professionals focused on their mission to ensure the well-being of vulnerable adults while supporting them to reach the goals of safety, health, dignity, and independence. While in this position, you may spend up to half of your time supporting the following programs:

  • Adult Protective Services (APS) - Adult Protective Services accepts and investigates reports of suspected abuse or neglect involving older adults age 60+ and adults with disabilities ages 18- 59.
  • In-Home Supportive Services (IHSS) - In order to stay living safely at home, eligible older adults and disabled residents of all ages, can apply for support from paid in-home care providers. IHSS clients get help with day-to-day tasks that they cannot do themselves.
  • Linkages - Linkages Social Workers create a plan based on goals identified to help improve the lives and well-being of seniors and older adults during a 6 month to 1-year period. Social Workers will stay in contact by telephone and make in-home visits.
  • Public Administrator, Public Guardian, and Public Conservator - We receive referrals for public administrator, guardian or conservator from a designated agency. Please speak to your medical or mental health provider if you or your loved one would benefit from having a court-ordered conservatorship.
  • Home and Community Based-Services (HCBA) Waiver Program - This program permits HCBA services that assist Medicaid beneficiaries to live in the community and avoid institutionalization.

Minimum Qualifications

Education and Experience:
One year of experience equivalent to the Sonoma County class of Public Health Nurse II.

OR

Two years of public health nursing experience obtained after receipt of a California Public Health Nursing Certificate.

AND

Possession of a California State Public Health Nursing Certificate

Driver's License: Possession of a valid driver's license at the appropriate level including special endorsements, as required by the State of California, may be required depending upon assignment to perform the essential job functions of the position.

The position advertised on this announcement requires possession of a valid California Driver's License.

Working Conditions: Duties may require exposure to hostile persons or persons with a mental health condition, and/or confrontational interpersonal situations; intermittently sit at a desk for long periods of time while reviewing patient charts and histories and preparing patient charts and reports; intermittently walk, stand, bend, climb and reach while making field visits, conducting clinics or making educational presentations or demonstrations; twist to reach equipment surrounding desk or treatment areas; perform simple grasping and fine manipulation; use telephone and write or use a keyboard to communicate through written means; see and hear with sufficient acuity to examine and assess patient conditions; lift moderate weight; may work in dangerous, high crime areas of the community; and/or standby, callback and work during non-routine hours.


Knowledge, Skills, and Abilities

Considerable Knowledge of: Modern principles, methods, and practices of public health nursing, including preventive medicine, epidemiology, public health education, gerontology, and sociological problems involved in public health nursing; principles and practices of public health program planning including environmental, social, physical, and psychological issues; Federal, State and local laws and regulations governing public health and nursing practices; community aspects of public health nursing programs. Procedures and practices used to promote maternal and child health and to assess infant, child, adolescent development and other special programs; principles and practices of work safety, especially as related to communicable diseases, differing cultural, religious, economic and social groups and their relationships to the delivery of and acceptance of health care services; and various preventive and early diagnostic techniques.

Working Knowledge of: Principles and practices of supervision; techniques of organizing and conducting classes and training sessions on a variety of health subjects; services and functions of other social and health agencies and providers relative to public health nursing; and demographic, bio-statistical and data collection methods utilized in evaluating public health needs and problems.

Ability to: Plan, control and direct the activities of nursing and other staff in an specialized public health nursing program area; train, assign, review, evaluate, and lead oversee the work of subordinate staff, consult with other staff and/or manage complex cases requiring advanced public health nursing skills and knowledge, assist in developing and implementing policies and procedures, assist in the evaluation of public health nursing program effectiveness, determine abnormalities in laboratory test reports; ability to differentiate medical problems that require immediate consultation with the physician rather than a deferred consultation; follow established protocols set up by physicians and to consult with physicians and other medical personnel; identify community health needs through analysis of demographic and bio-statistical data and information, write and speak effectively and prepare and maintain clear and concise records and reports; develop and maintain effective public relations with clients, staff, community groups and organizations; exercise independent judgment, analyze and evaluate situations independently and take appropriate actions; instruct in the prevention of disease and promotion of public health practices; work with various cultural and ethnic groups in a tactful and effective manner; use a computer, calculator, typewrite, telephone, facsimile machine, photocopy machine, and audio-visual equipment; and use clinical and instructional supplies and equipment including blood pressure cuff, stethoscope, audiometer, syringes and needles.
