Job Summary

The Ambulatory Care RN is responsible for providing professional nursing care through telephonic triage, clinical assessment, care coordination, and direct support to ambulatory health programs. This role ensures timely clinical evaluation, prioritization of patient needs, and adherence to nursing scope of practice, clinical protocols, and patient safety standards.

The RN functions as a clinical decision-maker, utilizing evidence-based triage protocols, nursing judgment, and organizational guidelines to assess patient needs, determine appropriate levels of care, and facilitate safe and effective patient outcomes.

Essential Job Functions

Provide telephonic triage services in accordance with established clinical protocols, nursing scope of practice, and organizational policies.

Assess patient symptoms, severity, and urgency using standardized triage guidelines and nursing judgment.

Provide clinical advice, education, and recommendations within scope of practice.

Escalate urgent or emergent cases appropriately, including directing patients to emergency services when indicated.

Coordinate care with providers, behavioral health teams, pharmacies and other departments to ensure continuity of care.

Coordinate prior authorizations and escalate denials or required clinical changes to providers.

Document all triage encounters accurately, timely, and thoroughly in the electronic medical record.

Schedule urgent, same-day, and follow up appointments based on clinical assessment and provider guidance.

Provide clinical nursing support to physiatry clinics, including assistance during botulinum toxin (Botox) injections.

Assist with clinical oversight and guidance of medical assistants and support staff as needed.

Formal Education and Job-Related Experience

Graduate of an accredited school of nursing. Bachelor's degree preferred.

Experience in pediatric nursing or ambulatory care is preferred.

Certification

American Heart Association BLS Provider Certification required.

License

Current New Jersey RN license.
