About the Role

The Williams Care Manager - Assessment Clinician is more than a clinician-you are a connector, advocate, and change agent. In this role, you'll guide individuals through one of the most important transitions of their lives, coordinating care, conducting assessments, and collaborating across systems to ensure long-term success.

What You'll Do

Care Coordination & Transition Planning

  • Lead and coordinate transitions from nursing facilities to community placements
  • Develop and implement individualized, person-centered transition plans
  • Collaborate with multidisciplinary teams including medical providers, housing specialists, and recovery supports

Clinical Assessment & Support

  • Conduct comprehensive clinical assessments
  • Provide ongoing clinical services before, during, and after transition
  • Apply trauma-informed, strengths-based, and recovery-oriented practices

Collaboration & Advocacy

  • Serve as a key liaison between nursing facilities, community providers, and state partners
  • Represent the agency in case reviews, meetings, and system discussions
  • Build strong partnerships to ensure continuity of care

Compliance & Documentation

  • Complete and submit all required reports accurately and on time
  • Track consent decree benchmarks and ensure compliance
  • Maintain high standards of documentation and accountability

Continuous Improvement

  • Participate in trainings and program development efforts
  • Support data tracking, outcomes reporting, and quality improvement initiatives

Education & Compensation
  • Bachelor's Degree: $25.00/hour + Every Other Friday Off
  • Master's Degree: $60,000 annually + Every Other Friday Off

Additional compensation available for certifications/licensure: CADC, LPC, LSW, LCSW, LCPC
