• 1+ years of LPN experience
• LPN license
• Current CPR & PPD
• Able to lift 50 lbs.

Job Responsibilities

• Medication Administration: Administers routine oral, subcutaneous, and intramuscular medications, as well as treatments, following nursing protocols and facility policies.
• Health Monitoring: Observes, records, and reports changes in the health status of individuals to medical providers, including managing chronic conditions and identifying potential acute issues.
• Documentation: Maintains accurate, detailed medical records, including Medication Administration Records (MAR), AIMS/DISCUS evaluations, and swallowing assessments.
• Daily Care: Assists with personal care, hygiene, and daily activities to ensure a safe, sanitary, and therapeutic environment.
• Safety and Support: Protects individuals from harm and assists in responding to emergency situations.
• Coordination: Assists physicians and RNs with physical examinations and treatments.
