Overview

The SC Float Social Worker identifies the need for medically related social services and supports the provision of these services across assigned skilled nursing communities. This position provides coverage as needed and is responsible for identifying the psychological, social and emotional needs of residents and implementing appropriate services and interventions. This role includes collaboration with interdisciplinary teams and outside community resources to ensure continuity of care during periods of transition, vacancy, or increased need.



Responsibilities

    Provides short-term and transitional social service support and advocacy for residents/patients and their families across assigned skilled nursing communities.
  • Identifies and supports residents' individual needs, preferences, customary routines, concerns, and choices through assessments and care planning, ensuring interpreters are utilized when indicated for those who do not understand English.
  • Identifies options and interventions that best meet the physical, emotional, and psychosocial needs of each resident/patient during the coverage period.
  • Assists residents/patients and families with financial and legal matters as appropriate, including POA, applications for Medical Assistance, and referrals to legal or funeral planning resources.
  • Participates in discharge planning and the development and implementation of social service care plans, including coordination of services for residents with behavioral and/or psychological needs.
  • Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect, and Justice by integrating these principles into day-to-day activities and interactions.
  • Performs other duties, tasks, or projects as assigned to support continuity of social services during the temporary assignment.


Qualifications

QUALIFICATIONS REQUIRED

  • Bachelor's Degree from a school accredited in the Council of Social Work education
  • Licensed by the Board of Social Work in the State of practice, which must be in good standing

QUALIFICATIONS PREFERRED

  • Hospital or nursing home experience
  • Experience with geriatric population
