Directly reporting to the Vice President, Ambulatory Nursing and Home-Based Services, the Performance Manager will work collaboratively with the members of the ambulatory nursing, home based and post-acute care services leadership, management and medical staff leadership. They are responsible for providing direction, planning, implementation, evaluation and consultation to ensure that all priority programs are performing successfully. This involves a high level of both project management and data analysis, display, interpretation. It requires working to improve communication, key processes, policies, procedures, protocols (care pathways) across the enterprise. They are expected to be a collaborator, influencer and improvement advisor regarding priority ambulatory operations. They will be expected to work closely and collaboratively with the Ambulatory Operations Leadership, particularly the sections of therapeutic infusion, clinical triage and ambulatory nursing operations. This work requires extensive knowledge, skills and expertise in complex change and project management. This position will manage a diverse group of clinical and non-clinical staff in order to assist the organization in achieving success in mission critical initiatives that improve the lives of the patients we are privileged to serve while continuously improving our financial position.

Responsibilities

  • 1. Manages and leads efforts to monitor and systematically improve performance of the therapeutic infusion, clinical triage and other ambulatory nursing priorities as assigned.
    • 1.1 Manages Interact Program through Qualidigm to understand referral and transfer patterns of the preferred network.
  • 2. Under the direction of the Vice President, Ambulatory Nursing and Home Based Services and in collaboration with the Ambulatory Operations team, manages the mechanisms of performance including but not limited to collaboratives, councils, data dashboards, process improvements.
    • 2.1 Develop yearly PI plan with input from network, stakeholders and effectuate such plan (including necessary updates to contractual agreements) over the course of the fiscal year.
  • 3. Enhances knowledge, breadth and depth of ambulatory nursing measures and projects across the enterprise.
    • 3.1 Attend meetings, visit sites, read and report on care continuum activities in the healthcare environment.
  • 4. Regularly rounds and audits clinical practices to assess performance against applicable goals and requirements.
  • 5. Collaborates with the practice optimization team to maximize efficiencies of work improvements.
  • 6. Performs other assignments as directed and/or required by the Vice President, Ambulatory Nursing and Home Based Services, Chief Ambulatory Officer and/or Chief Ambulatory Physician.

Qualifications

EDUCATION

Masters in Nursing, Public Health, Business and/or Administration strongly preferred. Professional healthcare licensure or relevant healthcare certification required (RN, PT, OT, SW or CPHQ, CHES, PMP, etc.).

EXPERIENCE

Three to five years' experience in healthcare environment, including at least 2 years in project management or process improvement. At least two years in managerial/program supervisory experience.

LICENSURE

Professional licensure such as Registered Nurse, Physical Therapist, Social Worker with valid CT license, or eligible or holds specific certification making qualified - particularly a PMP.

SPECIAL SKILLS

Excellent interpersonal, communication and leadership skills. Ability to work collaboratively and independently. Demonstrated personal maturity and good judgement. Expertise in data collection, change management, process improvement, safety. Expert knowledge in information systems such as Helix and Epic and external databases such as Vizient.

PHYSICAL DEMAND

Ability to travel across state.
