THE POSITION

Lackawanna County Area Agency on Aging has an exciting opportunity for an individual who is patient, compassionate, and organized. We are looking for Aging Care Manager 1s that are interested in helping older adults discover resources that will enhance their quality of life. DESCRIPTION OF WORK

Within this role, you will be trained to assess the needs of older adults; develop and implement care plans; provide advocacy and ongoing care management; as well as protect consumers from abuse, neglect, exploitation, and abandonment. Our goal is to improve the quality of life for older adults by providing them with access to a range of community and home-based services. You will learn how to accurately complete state mandated assessment forms, respond to and investigate Reports of Need, develop effective care plans, and ensure care plans are properly implemented. This position gives you the opportunity to find meaning in your work by empowering and advocating for the rights of older adults while supporting their family and other caregivers. Additional responsibilities include:

  • Conducting home visits for safety and welfare checks
  • Serving as a liaison and mediator between consumers and providers
  • Making referrals to law enforcement and recommending cases for court intervention
  • Interviewing collateral contacts that may be present during the initial visit
  • Gathering information for investigations
  • Educating consumers and their families regarding aging services and benefits
  • Encouraging enrollment in other community resources

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Three years as an Aging Case Aide 2; or
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

  • Hiring preference for this vacancy may be given to candidates who live within Lackawanna County. If no eligible candidates who live within Lackawanna County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
