Scope of Work:
The Social Work Navigator assesses each patient's needs and develops an appropriate care plan. This position applies evidence-based practices to support individuals facing mental health challenges. Serving as a link between acute care and community agencies, the Social Work Navigator oversees a caseload of patients, provides therapeutic interventions, and manages psychological care for this population. The role collaborates with interdisciplinary team members in both acute and community environments to help patients and families navigate the process of securing mental health services. Interventions may include outpatient psychiatrist appointments, supporting medication adherence, facilitating referrals, identifying care goals, coordinating access to crisis centers, and working with community mental health and local mental health providers to promote safe transitions.
  • Support patients and families in navigating behavioral health services and resources across acute and community settings, using in-person, electronic, and telephonic communication to bridge care gaps.
  • Conducts comprehensive assessments to determine health requirements, care preferences and social determinants, providing navigational support to facilitate appropriate care solutions.
  • Utilize clinical interviewing skills to elicit patient information and determine appropriate interventions.
  • Accurately completes and maintains clinical documentation in the electronic medical record, reflecting assessments, care plans, and transition needs.
  • Provides consultation and resources to members of the healthcare team on behavioral health placement options and care coordination strategies.
  • Serve as a liaison between patients, families, healthcare providers, and external agencies to ensure continuity and clarity of care delivery.
  • Deliver short-term therapeutic assistance suited to the specific setting and location.
  • Stay informed on current community resources, behavioral health regulations, and participate in relevant committees or initiatives that support role development internally and externally.
Qualifications
  • Required Master's Degree Social Work
  • 3 years of relevant experience in health care following completion of a Master's Degree in Social Work, with a focus on care management preferred. Required
  • Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning. Preferred
  • Master Social Worker (MSW-Master) - State of Michigan Upon Hire required
