Description 
Nexus Family Healing is hiring a Care Coordinator at our Onarga, IL location! Nexus-Onarga Family Healing is located approximately 30 miles south of Kankakee and 45 miles north of Champaign-Urbana, just a few miles off of I-57 on a historic site in Onarga, IL. Nexus Onarga provides boys ages 10 to 20 years old a structured, homelike environment where they can safely address a variety of unhealthy behaviors and mental health issues as well as childhood trauma.   - Schedule: Tuesday-Saturday or Sunday-Thursday based on the program needs
 - Work hours range from 7a-7p based on the needs of youth & families served
 - Salary starts at $25.20-$27.00 per hour based on prior experience & education
 
  
 Work Environment: 
 
  - Must be able to lift to 50 pounds unassisted and up to 100+ pounds with assistance.
 - Must be able to stand and walk for extended periods of time (2-4 hours).
 - Must be able to twist, bend and stretch in a manner conducive to the execution of daily activities within the residential program.
 - Must be able to assist in proper restraining of client.
 - This role routinely uses standard office equipment such as desktop and/or laptop computers, photocopiers, and smartphones.

 The Care Coordinator supports treatment services by guiding clients and families, coordinating resources, and ensuring continuity of care for stabilization. Primary responsibilities: 
 
  - Provides mental health counseling to assigned youth and documents services, meeting or exceeding minimum standards.
 - Manages and coordinates resources and activities for each assigned client(s) to ensure that the unique treatment needs, and requirements of each individual are met.
 - Facilitates client and family engagement in treatment, identifies barriers, and connects them with support through transition or discharge.
 - Participates in and/or facilitates all required meetings and conferences relative to assigned clients.
 - Communicates clearly and respectfully with clients, families, staff, and external contacts; shares knowledge and resources appropriately.
 - Maintains required documentation and current files for all clients, meeting organizational and regulatory standards.
 - Works effectively with internal stakeholders to successfully implement plans for achievement of treatment objectives and community reintegration.
 - Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients.
 - Participates in the training and development of other care coordinators and assumes mentorship role as assigned.
 - Demonstrates the knowledge and understanding of all Nexus policies and procedures and ensures compliance with the ability to reference and use them.
 - Demonstrates concern and empathy towards others as needed, while maintaining appropriate boundaries.
 - Exhibits a high degree of ethical conduct, integrity, and confidentiality, while adhering to the Nexus Code of Conduct.
 - Demonstrates self-awareness and manages emotional responses professionally in daily work situations.

Requirements 
Required Education and Licensure: 
 
  - Meets the Mental Health Service Definitions and Professional Qualifications of a Mental Health Professional (MHP) 
 - Bachelor's degree in a clinical discipline (psychology, social work, etc.). In lieu of clinical degree, any bachelor's degree plus 2 years of related experience required. In lieu of bachelor's degree, will consider MHP with 5 years of related experience. 
 - Minimum 21 years of age.
 - Valid driver's license required. Must meet state regulating agency and Home Office driving requirements 
 
  
 Preferred Education and Experience: 
 
  - 3 years of related experience.
