Description

Homebase Medical is expanding rapidly into the Everett / Lynwood areas. We are currently searching for an experienced Palliative Care Nurse Practitioner. You will be responsible for palliative care in the home setting.

Key Duties:



  • Conduct Palliative Care Visits
  • Comprehensive Assessment skills including pain management, symptom management, patient and family education.
  • Focus on goals of care.
  • Work collaboratively with our interprofessional palliative care team
  • Support after hours, holiday and weekend on-call
  • Support provider relationships with our clients
  • Actively support the achievement of Homebase Medical's Vision and Goals.

Qualifications:





    • Active Nurse Practitioner license in the State of Washington
    • Board certification (Nurse Practitioner)
    • DEA licensure
    • 2 years or more of palliative care experience required
    • CAPC preferred but not required
    • Experience diagnosing and caring for Dementia/Alzheimer's patients.
