Pharmacist - Value Based Care Program Manager

HealthPartners is hiring a pharmacist to support Value Based Care.

This position will focus on the following:

  • Serve as a strategic thought partner, advisor and executor of value-based care (VBC) pharmacy programs and interventions for HealthPartners care delivery. This position will be a critical member of the VBC Core Team, which includes executive sponsors, key stakeholders, IT, contracting, Finance, Data Ops & Health Informatics & Diagnostic Accuracy.
  • Promote cost-effective prescribing through interdisciplinary collaboration including clinician education and targeted outreach based on analysis of prescribing trends.
  • Analyze and support data development to identify areas of improvement in medication management and contributing to the development of VBC initiatives.
  • Oversee the development and monitoring of VBC pharmacy quality metrics in alignment with overall VBC measures of success.
  • Develop comprehensive medication management tools and collaboration with other healthcare professionals to optimize medication regimens and improve adherence through transitions of care.
  • Lead population health and quality improvement clinical pharmacy programs designed to optimize disease state care through coordination of care, and companion diagnostics.
  • Align prescribing payer-preferred products for Value Based Payer contracts, advise on contract negotiations for VBC pharmacy contracts, and ensure contracts support financial and value-based goals.
  • Note this position reports up through Pharmacy leadership with adjacent reporting to Value Based care leadership.

MINIMUM QUALIFICATIONS:

  • Education, Experience or Equivalent Combination:
    • Bachelor's degree in pharmacy or Doctor of Pharmacy degree from an Accredited College of Pharmacy
    • 5 years Pharmacy experience, Hospital or Ambulatory
    • 2 years experience in program and/or project management
  • Licensure/ Registration/ Certification:
    • Current Pharmacist License in the State of Minnesota
  • Knowledge, Skills, and Abilities:
    • Able to assess and utilize business data to detect opportunities and issues; strong analytical thinking.
    • Excellent verbal and written communication skills
    • Excellent problem-solving skills
    • Excellent organizational and project management skills
    • Demonstrated ability to maintain effective relationships
    • Demonstrated ability to work independently
    • Proficiency with Microsoft Office software

PREFERRED QUALIFICATIONS:

  • Education, Experience or Equivalent Combination:
    • Completion of a Pharmacy Residency
    • At least 5 years of experience in value-based care, managed care, formulary management, academic detailing to providers, or similar roles
    • Understanding of VBC models, incentive structures, risk-based contracts, and relevant programs
    • Ability to lead teams
    • Demonstrated ability to effectively communicate, persuade, and negotiate with others.
    • Detailed knowledge of Epic and basic understanding of clinician workflows
  • Licensure/ Registration/ Certification:
    • Certification by the Board of Pharmacy Specialties.
    • Any specialized certifications/certificates which enhance expertise in areas crucial for value-based care models, such as quality improvement, informatics, leadership, and specific clinical areas like primary care, oncology, or critical care.
  • Knowledge, Skills, and Abilities:
  • Drive and standardize VBC Pharmacy initiatives across all care delivery (hospitals and clinics) as well as bridge the gap to expertise within our health plan to ensure we're delivering the highest value especially within our shared patient: member population.

ESSENTIAL DUTIES:

  1. (30%) - Provide strategic leadership, and drive execution of sustainable VBC pharmacy programs producing clinical outcomes in partnership with multiple stakeholders across the organization.
  2. (20%) - Develop and implement population health initiatives aimed at optimizing pharmacy quality, enhancing chronic disease management, improving medication adherence and promoting cost-effective medication use.
  3. (10%) -Develop and leverage data analytics to assess prescribing trends for both medical benefit and pharmacy benefit drugs; engage directly with clinicians to identify opportunities for reducing total cost of care while maintaining high-quality patient outcomes. Foster and sustain collaborative relationships with clinic staff, medical directors and internal departments.
  4. (20%) - Partner with HealthPartners and other Health Plan Clinical Pharmacy Programs to analyze medical and pharmacy benefit design, utilization patterns and identify strategies for improving clinical outcomes and reducing costs for patients and members.
  5. (10%) - Work jointly with IT professionals and clinicians to develop, innovate, and utilize tools within the electronic medical record that drive best practices in medication utilization and support optimal disease state management, ensuring all solutions are practical and patient-centered.
  6. (10%) - Actively participate and collaborate with Value Based Care Committees or work groups and fulfill additional responsibilities as required.
