MDS Coordinator - Nursing Facility

Part-time, first shift (Wednesday - Friday, 8:00a-4:00p)

Job Description:

The MDS (Minimum Data Set) Coordinator is responsible for the coordination of the Resident Assessment Instrument (RAI), the electronic transmission of the RAI data to the state, the transfer of Patient Driven Payment Model (PDPM) to the billing department and ongoing education of those preparing the MDS. Responsible to assure information is accurate and complete and submitted in a timely manner. Work collaboratively with the multidisciplinary team. Knowledgeable about CMS and JCAHO long term care requirements and assists manager in assuring compliance. Monitor costs and reimbursement for Nursing Facility (NF) patients/potential patients in collaboration with fiscal services staff. Screens patients for appropriateness of admission to NF and facilitates admission from acute care status to NF. In absence of manager, assumes leadership role for NF under direction of manager designee.

Requirements:

• Associates degree (ASN) in Nursing required.

• Current unrestricted RN license in the state of KY required.

Preferred Qualifications:

• Bachelors degree in Nursing preferred.

• Recent experience in long-term care preferred. Experience in MDS completion or other data set completion/submission preferred. Knowledge of financial implications for long term care patients preferred.
