The Scheduling Specialist is responsible for managing patient referrals and visit schedules according to the care plan and visit strings. Assigns patient visits and other visits as ordered by an appropriately licensed provider (MD, DO, or NP.) Using the online system in collaboration with the Scheduling Supervisor to identify clinicians with the appropriate availability, experience and skill set to match the patient's needs.

Education

High School education or equivalent

Experience

1 year scheduling experience with a homecare or hospice agency preferred.

Knowledge, Skills and Abilities

  • Knowledge of scheduling and data entry experience preferred.
  • Ability to coordinate many activities at one time.
  • Strong, empathetic customer service experience.
  • Excellent verbal and written communication skills
  • Computer Experience
  • Ability to adapt to changing organizational needs.

Physical Requirements

  • Possesses and maintains good physical stamina and mental health.
  • Ability to concentrate and pay attention to detail when performing basic tasks.

  • Works in office setting

Principal Duties and Responsibilities

  • Utilizes an automated scheduling system to maintain services for per patient visits, while keeping staff centrally located.
  • Processes workflow for requested, scheduled, missed, rescheduled, reassigned, declined, and completed visits
  • Monitors requests for visits from Case Managers and Team Managers and assigns promptly
  • Communicates daily with Scheduling Supervisor, Team Managers, and field staff regarding any visit string compliance concerns, any missed or incomplete visits.
  • Runs scheduling reports and completes requests as required by Medicare guidelines as well as monitors productivity to ensure equitable distribution of assignments, under Scheduling Supervisor supervision.
  • Communicates with patients and families to determine specific scheduling needs. Communicates any schedule changes to patients, families, and facilities to ensure good customer service.
  • Continually monitors assigned work queues completes assigned tasks, coordination notes, and administrative tasks in a timely manner.
  • Assists in the coordination of scheduling and tracking all Supervisory Visits as required per regulatory requirements.
  • Monitors assigned team calendars to cover visits during scheduled and emergent absences. Anticipates staffing needs and communicates with the Scheduling Supervisor to ensure adequate staffing to meet visit demands, inclusive of holidays and weekends.
  • May be asked to work in the capacity of a hospice aide, when needed.
  • Performs other job-related tasks and duties as assigned.

Pay Range

$16.85 - $25.59

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
