A Brief Overview: As a Diabetes Care and Education Specialist Certified, you will serve as a crucial resource in diabetes care, education, and support for patients living with diabetes. You will provide individualized diabetes education, facilitate self-management, and collaborate with a multidisciplinary healthcare team to enhance the quality of life for individuals with diabetes.
What you will do:
- Develop and deliver comprehensive diabetes education programs, including diabetes self-management, medication management, nutrition counseling, and blood glucose monitoring.
- Provide personalized counseling and education to patients with diabetes, addressing their unique needs, challenges, and lifestyle considerations.
- Stay updated on the latest evidence-based practices, guidelines, and advancements in diabetes care to provide current and accurate education to patients.
- Offer emotional and psychological support to patients and their families, helping them navigate the physical and emotional challenges associated with diabetes.
- Assist patients in understanding and managing their diabetes medications, including insulin therapy and oral medications.
- Educate patients on the importance of lifestyle modifications, such as diet, exercise, and stress management, in diabetes management.
- Collaborate with physicians, nurse practitioners, dietitians, and other healthcare professionals to develop and implement individualized care plans for patients with diabetes.
- Regularly assess patients' progress, adjust care plans as needed, and provide ongoing support to promote successful diabetes management.
- Participate in community outreach programs, workshops, and events to raise awareness about diabetes prevention and management.
- Maintain accurate and detailed records of patient interactions, educational sessions, and outcomes, ensuring compliance with hospital policies and procedures.
What you will need:
- Bachelors Degree Nursing Required
- 2 Years of clinical experience in diabetes management or a related field.
- Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education (CBDCE) Certified Diabetes Care and Education Specialist (CDCES) certification. Required And
- Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current licensure as a Registered Nurse (RN) Required And
- Registered Dietician (RD) - Commission on Dietetics Registration (CDR) Dietitian (RD) in the state of Illinois Required
