Position Summary:
We have an exciting opportunity to join our team as a Assistant, Director - Social Work/Discharge Planning.

Under the direction of the Director of Social Work, directs and manages Social Work programs at NYU Langone Health to support overall Social Work/Case Management Program goals. Collaborates with Social Work and Case Management leadership on operational goals and work flow to achieve optimal staff efficiency and proactive management of patient flow, psychosocial needs, and timely transition planning. Provides leadership in the development of programs and services to address patient/family needs. Ensures all Social Work services are well organized, properly directed, staffed and integrated within units and services in the hospital. Guides continuous review of the placement process through appropriate tracking and reporting software (Epic/CarePort), as well as measurement of Social Work Program effectiveness and patient (customer) satisfaction. Supports education and training associated with this program.

Job Responsibilities:

    • * Meets attendance and additional performance standards

* Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of duties

* Establishes SW program and departmental goals in accordance with the hospital mission, organizational goals and the needs of the patient/family population

* Manages staff, including direct reports and provides general oversight of the Social Work staff; assures overall staffing is effective to meet the needs of the hospital and patients served

    • * Is effective in communication and relationships with others

* Ensures that the quality and appropriateness of patient care services provided by the Social Work Department are monitored and evaluated, and that identified problems are resolved appropriately

    • * Plans, develops, and approves new programs and evaluates existing programs for change.

* In collaboration with Social Work and Case Management leadership, monitors and administers the medical centers discharge planning program ensuring that appropriate resources are available for the development of post-hospital discharge plans for patients that provide a safe transition and continuity of care.

* Manages the operational activities of the department and ensures the quality of Social Work services in the Medical Center including programs for performance improvement; establishes, approves and administers policies and procedures. Implements and communicates operational strategies and solutions in line with the department business plan and the Medical Center mission

* Participates in appropriate Medical Center activities and appropriate Hospital/Medical Board Committees to further hospital mission and organizational goals. Maintains interagency relationships that promote the interest of the Medical Center

* Ensures that staff are practicing proper infection control techniques, are knowledgeable of fire safety, and maintain a safe work environment

* Ensures departmental compliance with applicable standards of the Joint Commission on Accreditation of Health Care Organizations as well as applicable federal, state, and city regulations; designs and implements a comprehensive program of quality assurance/performance improvement

  • Demonstrates sound decision making and initiative. Is effective in planning and time management

Qualified candidates must be able to effectively communicate with all levels of the organization.

Minimum Qualifications:

Masters Degree in social work, or its equivalent, (eg. A higher degree from a social work program that is registered by the Department as licensure qualifying, accredited by the Council on Social Work Education (CSWE) or determined by the Department to be substantially equivalent to a registered program. Licensed Clinical Social Work (LCSW required; must complete coursework of training in the identification and reporting of child abuse in accordance with Section 6508(3)(a) of the Education L W. Credentialing in Care Management preferred (ACM, CCM). Excellent verbal and written communication skills and demonstrated leadership qualities required.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $95,637.60 - $134,400.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
