GENERAL STATEMENT OF JOB
The Clinical Pharmacist supports the provision of high quality and cost-effective healthcare for Medicaid, uninsured, and underinsured consumers in the Vaya region. Clinical and administrative supervision of this position is provided by the Vice President of Pharmacy Operations. The Clinical Pharmacist also works collaboratively with other medical team members including other clinical pharmacists, the Vaya PBM pharmacist, Chief Medical Officer and Deputy Chief Medical Officer, care management staff and nursing. Services provided by the Clinical Pharmacist include clinical chart review, interdisciplinary team huddles, education, consultation, and participation in Care Management and Quality Improvement projects. The Clinical Pharmacist will also assist in precepting pharmacy student rotations.
ESSENTIAL JOB FUNCTIONS
Pharmacy Consultations:
- Complete patient chart reviews and evaluate comprehensive medication lists submitted by nursing staff with documentation in the Vaya care management platform
- Review available paid pharmacy claims data and integrate findings into consultations
- Communicate resulting recommendations to the appropriate parties
- Assist in support and monitoring of Vaya nursing and care management staff in the process of accurate and thorough medication reconciliation for specific population groups
- Communicate directly with pharmacies and prescribers in clinically appropriate situations
Other Clinical Roles:
- Help provide medical team coverage of weekly interdisciplinary care manager huddles
- Provide pharmacist representation for Vaya committees such as Clinical Strategy Committee and potentially other external regional partner committees/meetings
- Assist pharmacy department in drug information evaluation
- Assist pharmacy department in evaluating and adjusting the care management pharmacist review criteria as appropriate
- Consult with nursing staff to support understanding of pharmacologic clinical best practices
- Assist in development of new and innovative Vaya clinical pharmacy programs to support Vaya's shift to a North Carolina Medicaid Tailored Plan
- Coverage of other duties, including new clinical responsibilities developed in the process of becoming a NC Medicaid Tailored Plan, as assigned
Teaching and Training:
- Collaborate with the pharmacy department, human resources and outside entities in the design and provision of pharmacy-related collaborative care training for regional staff and providers
- Work with pharmacy department on pharmacotherapy best practice updates/education provided to Vaya staff and other groups such as the external prescriber attended Clinical Advisory Committee
- Share in supervision of pharmacy student clinical rotations
- Provide input for development of competency requirements for nursing and care manager staff
Quality Improvement:
- Work with the pharmacy department on recommendations based on trends and incident data gained from consultations/data review for Quality Improvement Projects
- Participate as assigned in Quality Improvement Activities
- Provide support of Vaya accreditation activities
KNOWLEDGE, SKILLS, & ABILITIES
- Expertise in the clinical application of pharmacy practice and broad knowledge of mental health disorders, substance use disorders, developmental disabilities and of appropriate interventions for these disorders in addition to expertise in pharmacy practice related to physical health disorders
- Ability to work in a remote setting by setting priorities and working independently
- Awareness of and cultural sensitivity to the unique needs of the I/DD, TBI and BH population
- Awareness and understanding of populations at risk of experiencing health care disparities
- Ability to solve problems and deal with new variables
- Ability to provide clear and concise guidance with regard to pharmacy best practices and to consistently interact professionally with patients, prescribers, service providers and staff
- Proficiency at planning, organizing, conflict resolution, negotiating and interpersonal skills
- Confidence as a public speaker with willingness to provide presentations within the organization and to community partners and stakeholders.
- Proficiency with Microsoft Office applications including Word, Excel and PowerPoint and ability to navigate electronic record systems
EDUCATION & EXPERIENCE REQUIREMENTS
Doctor of Pharmacy degree (PharmD) from an accredited College of Pharmacy and:
- Have completed a psychiatric clinical pharmacy residency
OR
- Have completed a PGY1 pharmacy residency in another concentration and have 4 years experience that includes over 50% of clinical practice focused on behavioral health populations with diagnoses of severe and persistent mental illness, and/or substance use disorders, and/or intellectual/developmental disabilities or equivalent experience.
Licensure/Certification Required:
- Unrestricted pharmacist license from the North Carolina Board of Pharmacy
PHYSICAL REQUIREMENTS
- Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
- Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists, and fingers.
- Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
- Mental concentration is required in all aspects of work.
RESIDENCY REQUIREMENT: The person in this position must reside in NC, SC, GA, TN, VA, MD, or FL.
SALARY: Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
