Responsibilities
  • Registered Nurse Residency - Designed for new graduate RNs, this program provides a supportive transition into home health nursing. Residents receive hands-on training, mentorship, and structured learning experiences to build confidence in delivering patient-centered care in the home setting.

  • RN Case Manager - As a key coordinator of patient care, the RN Case Manager develops individualized care plans, collaborates with interdisciplinary teams, and ensures patients receive the right services for optimal health outcomes at home. This role involves direct patient visits, education, and advocacy.

  • RN Admission - Specializing in the intake process, the RN Admission conducts initial assessments, educates patients and families about home health services, and establishes care plans tailored to individual needs. This role sets the foundation for successful home-based care.

  • RN Visit Nurse - Focused on providing hands-on clinical care, the RN Visit Nurse completes scheduled visits to administer treatments, monitor patient progress, and provide education. This role ensures patients receive ongoing, high-quality nursing care in their homes.


Qualifications
  • Graduate of State Board approved program for Registered Nurses and valid license as RN in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
