Full time, 8:00 a.m. to 5:00 p.m., variable days and weekends.

Job Summary

Under the supervision of the Department Manager, coordinates the smooth intake of all admissions. Maintains communication with referral source, external case managers and insurance companies, and assists with the coordination of services.

Essential Functions
  • Ensures completion of all necessary processes to facilitate patient intake.
  • Serves as a liaison/contact person between acute care hospitals, the community, and facility programs. Notifies physicians and/or program directors, as appropriate, regarding referrals and potential admissions. Responds to all inquiries and identifies alternate community resources to address presenting needs when facility is unable to meet the need.
  • Assures medical orders from appropriate source are complete, arranges for medical equipment, pharmacy, and supplies prior to new admissions as appropriate.
  • Manages ongoing contact with payer as necessary and obtains continued stay approvals with the business office. Manages the documentation of communication and shares this with the team.
  • Assures that facility is ready to meet the patient's needs through coordinating arrangements for care plans among hospitalstaff, the physician, the patient, the family, other community resources, and staff. Prompts team to arrange for training ofstaff regarding unfamiliar procedures or problems pertaining to the patient's care, coordinates visits of staff to the hospital for patient conferences and communicates information regarding a patient to staff.
  • Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
  • Performs other duties as assigned.
Qualifications

Required

  • Associate's degree
  • 5 years of relevant experience, a rehabilitation or acute care setting

Preferred

  • Bachelor's degree
  • Prior experience with persons requiring long term acute care, sub-acute care, long-term care, home care and/or hospice.
