Job Details The Oncology Patient Navigator works in collaboration and continuous partnership with chronically ill or high risk patients to coordinate care between diagnosis, treatment, survivorship or end of life. He/she participates in the assessment and ongoing support of SKCC patients in close collaboration with the Care Team(s) s/he is supporting.ESSENTIAL FUNCTIONS: Serves as the point of contact, advocate, and informational resource for patients, care team, family/caregiver(s). Promotes timely access to appropriate care such as helping to schedule appointments Gathers records, organizes and supports Tumor Boards and Multidisciplinary Clinic(s) and preparation for effective oncology appointments Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s) Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patient's electronic record Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
