Position Summary
The Social Work Aide III assists clients transitioning from incarceration to their return to the community by providing appropriate assessments of need and associated community resources and social services to make the transition successful. Responsibilities include making recommendations for supportive services and enhancing the conditions of the client by providing information and education; connecting clients with community resources; and making home visits and follow-ups. In addition, this position acts to help clients, as a mediator, and to utilize and obtain needed services at MSHC-LP Johnson clinic, specifically for individuals recently discharged from the Winnebago County Justice Center. The individual will work at both the MSHC-LP Johnson clinic and the Winnebago County Justice Center to assist with care coordination. The ability to obtain a security clearance will be necessary for employment. The unpredictability that can occur from day to day requires the individual to be able to adapt rapidly to change by making adjustments in work schedules, activities, and proper allocation of resources.

Duties & Responsibilities
  • Evaluates client needs to determine appropriate community resources and social services to clients of all ages, including assessments, ongoing case planning, follow-up and home visits when deemed necessary.
  • Prepare care plans describing the services the client may need and use and the method of assessing medical and/or behavioral health treatment and needed social services.
  • Make recommendations to transferring facility to ensure necessary and appropriate services are arranged; provides resources and follow-up services within a multidisciplinary team of providers.
  • Educate patients with information and resources and assist with support services such as disabilities (SSI), food pantries, state ID, homeless shelters/housing, child care assistance programs, home health services through the Department of Aging, parenting classes, transportation, mental health and substance use treatment, and domestic violence services etc.
  • Make recommendation for health marketplace enrollment and assist patients in the health marketplace enrollment process requiring certification as a Certified Application Counselor.
  • Assembles and maintains necessary statistical information for administration including but not limited to daily work logs.
  • Maintain complete and accurate files on all client programs.
  • Participate in local coordinating committee meetings, interdisciplinary meetings, and training programs.
  • Represent MSHC-LP Johnson clinic at community events during regular and off hours.
  • Certified in outreach and enrollment of insurance.
  • Perform other related duties and participate in special projects as assigned.


  • Minimum Qualifications

    • Two years of work experience comparable to a Social Work Aide II
    • Any combination totaling two years from the following categories:
      • work experience which provided knowledge of human behavior, including the ability to cope with a variety of people and/or understand the importance of people's needs and feelings, in a public/customer service environment.
      • college course work in social or behavioral sciences or related field, measured by the following conversion table or its proportional equivalent:
        • 15 semester hours equal six months
        • 30 semester hours equal one year
