The LPN- Long Term Care job provides individual and family-centered nursing care and treatment to pediatric, elderly, acutely ill, or injured residents. Integrates technical skills and theoretical knowledge to meet the individual needs of residents and families. This role assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. This role provides direction, education, and begins to assume leadership responsibilities at the unit level. This role delivers and accurately documents direct, age-specific care to a resident or group of residents based on medical orders and an integrated, multidisciplinary plan of care. This role also safely administers prescribed medications, identifying incompatibilities and/or interactions.
Licensure, Certification, or Registration Requirements for Hire: LPN licensure in Virginia or eligible
Licensure, Certification, or Registration Requirements for continued employment: Current LPN Licensure in Virginia AHA BLS Certification or equivalent
Experience REQUIRED: N/A
Experience PREFERRED: Experience in a long term care setting such as a nursing facility (NF), skilled nursing facility (SNF), or assisted living (AL).
Education/training REQUIRED: Graduate from a Board of Nursing approved School of Practical Nursing

Independent action(s) required: Follows documented physician/licensed independent provider orders, standards, and practice guidelines such as treatments and organizes work to carry out assignments. Stays alert to residents' condition and brings concerns to the attention of the RN immediately.

Additional position requirements: Licensed Practical Nurses may work all three shifts, weekends and holidays depending on work location. Individual shift rotation and schedules will be established by specific Nurse Manager/Practice Manager. Schedules, including shift rotation requirements and hours of work, may be adjusted as necessary to meet unit requirements and/or as needed.
