Short Description

  • We are seeking a dynamic and highly motivated team member who will serve as an essential part of the multidisciplinary team, the Lead Patient Navigator fulfills the gap in patient care between the clinical and administrative team.
  • The Lead Patient Navigator supports patients and their families during the coordination of care by completing intake calls, coordinating pre surgical testing and consultations as requested under direct supervision of the Surgeon and Nurse Navigator assigned.
  • Ensures surgical patients are optimized for upcoming surgery including but not limited to collecting and reviewing that necessary testing is completed, medical release and financial authorization is on file, and confirms patients are prepared for upcoming surgery.
  • Provides guidance through the surgical navigation process and provides concierge level customer service to ensure patients receive pertinent information for surgery and expectations are met.
  • In conjunction with the surgical scheduler and with direction of the nurse navigator, the surgical nurse liaison is responsible for both clinical and non-clinical duties as it relates to the surgical process.
  • Acts as a liaison between the patient/family, surgeon, nurse navigator, surgical scheduling, and authorization team

Experience Required

  • Required 0-2 years of experience: 3-5 years of experience preferred

Education Requirements

  • Required High School Diploma
