Title: Care Manager

Duration: 3 Months with potential to extend/convert

Location: - Remote and Member Facing - Osceola, Orange, Seminole FL

8am-5pm, Monday thru Friday

Based on tier of member, leadership may ask for them to meet with the member to connect with family, (NOT CLINICAL) more so a supportive liaison.

Quarterly meetings held at in office location 6801 W Colonial Drive, Suite E Orlando 32818

Day to day responsibilities:

Required to complete daily outreach calls based on member specific tiers
Outreach calls are to check in on the members, follow up on their needs, remove any barriers to care for the members.
Based on members needs the call volume will vary 5-20 calls per day
2+ years of social work experience or licensed mental health counselor in an acute

care or community setting. Knowledge of government sponsored managed care programs preferred.
Perform care management duties to assess, plan and coordinate all aspects of medical

and supporting services across the continuum of care for select members to promote quality, cost effective

care.

Performance expectations/metrics:

Compliance with care management policies/procedures including outreach and documentation expectations.
Individuals will be audited monthly to ensure charts are in compliance.
KPI metrics (are they meeting their monthly/quarterly outreach/ completing care plans, completion of annual assessments)
Face to face visit outreach

Required Skills/Experience:		Preferred Skills/ Experience:
1.	Pediatric case management experience (2 years)	1.	Health plan experience
2.	Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.	2.	Foster care and/or adoption experience
3.	Behavioral Health Experience	3.	Bi-lingual
Education Requirement:	Bachelor's Degree	Education Preferred:	n/a
Software Skills Required:	Microsoft Office- Outlook, Word, and Excel
Required Certifications:	Social Work Licensure, Licensed within the State of Florida or Compact Licensure	Required Testing:	n/a
